I know I have asked this question before but I can't remember the answer or find the orginal thread. My manuscript was previously copyright in 2004. Well I am finally about to self publish and I need to have that info updated in the copyright office, do you know the name of the form I am suppose to complete?

I'm assuming you mean registering the copyright with the US Copyright Office? I wouldn't bother, myself. You don't NEED to register the copyright - your work is automatically (c) you the moment you create it. Nor (IIRC, although IANAL) do you need to re-register an unpublished work when you are about to publish it. Check out http://www.copyright.gov/circs/circ1.html#cr
