Book with a companion CD?

I've successfully printed books with BookSurge and Lightning Source, but the current project (a book of children's plays) involves a companion CD and I'm trying to find a way to print it. The fallback plan is to take delivery of the books and then put the CD into a stick-on sleeve inside the back cover, but that means no drop shipping and having to handle each book. If I go with a major POD printer there's also the problem of them automatically listing the book (without the CD) and selling an incomplete product.

So does anyone know of a POD printer that can handle a CD? First choice would be having them do the sleeve insert, but it might be easier to find a company that can do a CD in a jewelbox and package the two under a single SKU/ISBN. So far I'm coming up empty on both plans.

I've looked into this a little. My conclusion was that the better option was setting up a website with the content available for free and including the url in the book. I'm also putting together the DVD for sale separately at low cost, for people that don't want to or can't download the content. There are lots of advantages: 1) no special book printer requirements 2) can update the content easily 3) website advertises book, helps sales.

Thanks. We thought about doing something like that, but I'm afraid most customers are going to need this as a physical CD. It's the sound effects for a collection of Sunday school plays and teaching guide. I'm afraid that most of the buyers aren't going to have the tech skills to download and burn it. The individual scripts should also be available in digital form for easier printing, but I think the CD will have room for both. I also have the impression that a book with a disc has a higher perceived value that would more than make up for the cost of including it.

I'm getting into a bind with the author needing to show up at a conference at the beginning of March with a stack of books. Leaning toward using a disc duplicating service and a local printer if I can't find a great solution or at least a POD house than can print without an automatic Amazon listing.
