About the "Foreword"

Hello,

What is the proper format, please, for 'Foreword' in an autobio/memoir ?

What I am specifically asked about is, do we put "Foreword" at the top of page, then the text, and then "John Smith" or do we include the writer of the forewords title like American journalist, educator, author, and activist ?

Not sure exactly what you're asking. Is it how do you display the author's name?

If that's the case:
On the title page of the book you put
Title
your byline
Foreward by x

In the table of contents if you have one, you write
Table of Contents
Foreward by x
then the titles of the rest of your book

For the Foreward itself: I've seen it done several ways, but the way it is in my yoga book is
Foreward
byline with x's name
text
brief note about x if you need it
