Submitting and Pen Names

Hi, when submitting short fiction, do we put our pen names on the cover letter and submission or our real names? I don't want them to use my real name, but I'm not sure how to tell them. It isn't included on the submission form.

Your cover letter and any correspondence uses your real name. The information at the top of the first page of the formatted manuscript gives your real name, address, etc. Then you have the story's title and "by Pen Name" in the manuscript's body. Your header will number the pages with a one-word version of your title, your pen name, and your real name: Pen Surname (Real Surname), TITLE, page number. Example: Twain (Clemens), SAWYER, 74

Publishers are used to this and it'll be clear to them.

Just for clarity, you never have to actually write the words "pen name" anywhere. Publishers automatically release a story by whatever name you write in the byline.

Hiding in this box I made.
I do the Real Name writing as Fake Name thing when I submit and, of course, just use my pen name in the byline on the actual manuscript.
